how do I close the gap on an "out of network" health insurance claim?


I was shocked to see my insurance company only willing to reimburse 25% of an MRI charge from an out of network facility. Calling an in-network facility, I found that they charge a similar amount and take a 75% write off. I am shocked that insurance companies and in-network doctors are in the practice of setting these guidelines. I was under the impression that for a PPO, you have an option to go out of network and play slightly more ... not 4x!
Any suggestions? Am I sunk or can I should appeal to the insurance or hospital?

Well, you do have the option to go out of network, but it's certainly not "slightly" more.

You're sunk. If you deliberately went out of network, you're going to have to pay the balance.
